Submitting for Reimbursement

Instructions – READ THIS

Thank you for your dedication and generous time you commit to Giáo Lý!  Our parish plans and budgets for the costs of our classroom books, supplies, and expenses.  We know many of you are very generous in donating to the parish.  At the same time, we want you to submit for reimbursements!

Submitting for reimbursements is GOOD!  Reimbursements help the parish know the true cost of the Giáo Lý program.   There are two ways to get reimbursed:

  • In-Person Paper Form:  
    1. Bring all your expenses to the parish.
    2. Find Sr. Mai at the parish office on Saturday, and she will give you a paper form.
    3. Fill out the paper, and give it to the director to sign.
    4. You will then submit your paper form and all your receipts to the Parish Office.
    5. A check will be mailed to you.
  • Or, use the online submission + turn in any paper receipts: 
    1. Fill out the electronic form below, and upload your receipts.
    2. You will receive a confirmation email, and the director will also receive the submission.
    3. The director will review and make sure everything looks good.  Director will then forward to Cha for final approval and payment of the reimbursement on your behalf.
    4. A check will be mailed to you once everything is received.

    Take a picture of your receipts, or upload a PDF of the invoice (e.g. print the Amazon invoice to a PDF). Each file is limited to 4 MB. Please submit them as JPG, PDF, or PNG files.
